Requesting a Printed Official Transcript

Electronic transcripts are only available to individuals who attended after fall 2004 to the present.

  • If you attended before Fall 2004, please use this Transcript Request Form to order a printed paper copy of an official transcript. Transcript request forms MUST be signed. Mail or email completed form to the address(s) below.

  • To order an electronic copy of an official transcript, fill out an electronic transcript request form on Parchment: Electronic Transcript Request Form. You will be able to create/use a Parchment Document Ordering account on behalf of LMU.
