Requesting a Printed Official Transcript

Electronic transcripts are only available to individuals who attended after fall 2004 to the present.

• If you attended before Fall 2004, please use this Transcript Request Form to order a printed paper copy of an official transcript. Transcript request forms must be signed. Mail or email completed form to the address(s) below.

Electronic Transcript Requests (not available to students who attended before Fall 2004)

• To order an electronic copy of an official transcript, fill out an electronic transcript request form on Parchment. You will be able to create/use a Parchment Document Ordering account on behalf of LMU.
