Basic steps to the IRB Process

• Get the application from the website
  • http://www.lmunet.edu/curstudents/ORGSP/IRB
    
    
• Make sure you have the required signatures (when applicable)
  • Principal Investigator
  • Chair
  • Any co-Investigator
  • Faculty Sponsor
  
  
  
• Complete the mandatory CITI program for protection of human subjects.

• Include the following items with your protocol when you submit:
  • Consent form(s)
  • Copies of instruments to be used (e.g. surveys, questionnaires, tests, etc.)
  • Copy of CITI training completion certificate
