Basic steps to the IRB Process

Get the application from the website

Make sure you have the required signatures (when applicable)

• Principal Investigator
• Chair
• Any co-Investigator
• Faculty Sponsor

Complete the mandatory CITI program for protection of human subjects.

Include the following items with your protocol when you submit:

• Consent form(s)
• Copies of instruments to be used (e.g. surveys, questionnaires, tests, etc.)
• Copy of CITI training completion certificate

Hand deliver or send via campus or snail mail the completed package to:
